Quality of Life Assessment – Supporter

This page is for people reflecting on quality of life in the person they support. It can be useful if you want a more structured way to think about their daily functioning, health, connection, practical circumstances, and emotional wellbeing.

Like our other supporter pages, this page is for structured reflection and learning, not diagnosis.

Quality of Life Assessment - Supporter

Please keep in mind the person you are supporting, and think about their life over the last two weeks. Use your best judgment. If you are unsure, choose the answer that seems closest overall.

Choose one answer in each row. This form is for structured reflection about the person you support. It is not diagnosis.

Scoring and interpretation

This supporter version follows the standard WHOQOL-BREF scoring method for the 26 core items, using observer judgment rather than self-report.

Overall quality of life and overall satisfaction with health are shown separately, and the 4 core domains are shown using WHO-style 4-20 and 0-100 scores.
